Employer Forms
Certain forms are required for Occupational Health visits. If Grupo InmediateCare has a business agreement with your company, your company-specific forms may be available at the clinic; however, please check with your employer prior to your visit to confirm. Federal or state forms applicable to everyone may be provided at the clinic and completed during your visit to ensure a smooth registration process.
DOT Physical Exams
Our clinic provides certified DOT Physical Examinations for commercial drivers who are required to meet the medical standards established by the U.S. Department of Transportation. These exams are necessary for individuals who operate commercial motor vehicles and must maintain a valid Medical Examiner’s Certificate (DOT card).
During the exam, our certified medical examiner evaluates your overall health to ensure you are physically qualified to safely perform the duties of a commercial driver. The examination includes a review of your medical history, vision and hearing tests, blood pressure check, and a general physical evaluation.
Our goal is to provide efficient, professional, and compliant DOT examinations so drivers can stay on the road safely and meet all federal regulations.
